Guidelines on Recruitment

 

  1.    Upon Submission of Application for Employment
       (Before taking the COA Pre-employment Test):

 

    1. Application letter

 

    1. Personal Data Sheet (PDS) (CS Form No. 212, Revised 2017) (2 original copies)

      • Passport size ID picture
      • Government issued ID
      • With thumbmark

 

    1. Work Experience Sheet as an attachment to the PDS (2 original copies)

 

    1. Transcript of Records (TOR) (photocopy)

 

    1. Diploma (photocopy)

 

    1. Certificate of Training/s (photocopy) with the following conditions:

a. Must not be acquired as part of the school curriculum

b. Must be relevant to the position to be filled. Hence, a course outline may be requested by the HRMO to support the training certificate

c. Must be undertaken within the last five (5) years reckoned from the date of issuance of appointment


– For positions requiring relevant training 

 

    1. Photocopies of the following:

For Licensed Professionals:
(e.g. CPAs, Engineers, Architect, Teachers, etc.)

  Certificate of Registration issued by the PRC

  Board rating issued by the PRC

  Valid professional license issued by the PRC

 

For Lawyers:

  Certificate of membership in the Philippine BAR issued by the Supreme Court (SC)

  BAR rating issued by the SC

 

For CS Professional/Sub-professional:

  Certificate of eligibility issued by the CSC

 


  

  1.    Upon Initial Interview
       (After passing the COA Pre-employment Test):

 

    1. Bring the original copies of the following documents for verification of the photocopies submitted in Step I: 

      • TOR
      • Diploma

 

    1. Submit a fully accomplished affidavit on declaration of information on relatives in COA, residency status, acceptance of place of assignment and willingness to accept lower position level/salary grade (if applicable)

 

    1. Decision issued by the office/court/tribunal for appointee with decided administrative/criminal case (photocopy)

  

 

   Additional Requirements for Transferees:

 

    1. Last approved appointment (photocopy)

 

    1. Performance Rating/s in the previous position for one year prior to the date of assessment (photocopy)

  

 

   Additional Requirements for Reemployment:

 

    1. Clearance from previous employer (photocopy)

 

    1. Clearance from the Office of the Ombudsman (photocopy)

   


  

  1.    Upon Passing the Final Interview

 

    1. Have the initially submitted PDS notarized (2 original copies)

 

    1. Have the initially submitted affidavit notarized

 

    1. Original Authenticated Copies of the following:

For Licensed Professionals:
(e.g. CPAs, Engineers, Architect, Teachers, etc.)

  Certificate of Registration issued by the PRC

  Board rating issued by the PRC

  Valid professional license issued by the PRC

 

For Lawyers:

  1.    Upon Assumption of Office

 

    1. Certification of Assumption to Duty (CS Form No. 4, Revised 2018) (2 original copies)

 

    1. Oath of Office (CS Form No. 32, Revised 2018) (2 original copies)

 

    1. Sworn Statement of Assets, Liabilities and Net Worth (SALN) (2 original copies)

 

    1. Position Description Form (DBM CSC Form No. 1, Revised 2017) (2 original copies)

 

    1. Medical Certificate (CS Form No. 211, Revised 2018) (original copy)

Attachments (original copies of results of the following tests:

  Blood Test

  Urinalysis

  Drug Test

  Chest x-ray

 

    1. Valid NBI Clearance (original copy)

 

    1. Certificate of Live Birth
      (authenticated copy issued by the PSA or the LCR of the municipality or city where the birth was registered) (original copy)

 

    1. Marriage Contract/Certificate – for married employee
      (authenticated copy issued by the PSA or the LCR of the municipality or city where the marriage was registered) (original copy)

 

    1. Certificate of employment (photocopy)
      – for positions requiring relevant experience

 

    1. Decision issued by the office/court/tribunal for appointee with decided administrative/criminal case (certified true copy)

  

 

   Additional Requirements for Transferees:

 

    1. Last approved appointment (certified true copy)

 

    1. Performance Rating/s in the previous position for one year prior to the date of assessment (certified true copy)

 

    1. Copy of service record (original copy)

 

    1. Clearance from previous employer (original copy)

 

    1. Clearance from the Office of the Ombudsman (original copy)

 

    1. Authority to transfer from previous employer (original copy)

 

    1. Certificate of last salary received (original copy)

 

    1. Certificate of leave credits from previous agency (original copy)
